We work as a small team to make sure you get the best all around care and support.

On the care team you’ll work with:

  • A Community Health Worker – they’ll work with you to get referrals to local resources and act as an interpreter/translator between you and the rest of the team
  • A Public Health Nurse – working with you to help answer questions about breastfeeding and providing lactation consulting services
  • A Community Paramedic – helping you to monitor your health with care plans and  take vitals

What We Do:

During your time in the program, the team will:

  • Check your vitals to monitor your blood pressure and blood glucose
  • Provide you with breastfeeding and lactation support
  • Work with you to make sure you’re getting connected to local community resources
  • Conducting home and virtual visits
  • Providing basic education on pregnancy, mental health, self care and so on

Ideally you would join the program between your 28th week of pregnancy and 4 weeks postpartum so you have the best combination of prenatal and postpartum care and support.

We provide home visits and virtual visits for moms in the Rice County Minnesota area.

We believe that all women and families should have access to support no matter what. We want to be able to support you, help you manage your blood pressure and blood sugar and give your family the best possible start in life.

The only criteria necessary to be eligible is that you must have hypertension (high blood pressure) or diabetes and be pregnant or newly parenting mother.

This program is completely free to you.
